You can apply for Death & TPD, Death Only or Income Protection insurance cover up to $2 million. You can also select either Fixed Premium or Fixed Amount, however you cannot have a combination of both.
To apply for Death Only or Death and TPD insurance or change your cover, simply follow the steps below:
Step 1
Complete the relevant Member Application Form or Member Insurance Election Form. The forms are available for download (see Related content).
Step 2
Complete the OnePath Personal Statement, if required
(required for Personal Plan members; all requests in excess of cover of $750,000; or any yes responses to the questions in the Member Insurance Election Form).
Step 3
Send your form/s to us.
Send your original signed and completed forms via post to:

To apply for Income Protection insurance or to change your existing cover, you will need to complete our Member Income Protection Form (Industry Fund or Personal Plan) and the OnePath Personal Statement form if you are applying for over $5,000 per month cover or have answered yes to any question on the Member Income Protection Form. Forms are available in Related content or on the Forms and Publications page, and then send your completed forms to:

You may cancel your insurance cover at any time. To do so, simply advise us in writing, and send to:
