Want to DIY? Rolling over is as easy as 1,2,3!

  1. Complete the Transfer Between Funds Form, (see Related content at left).
  2. Provide a certified copy of identification (ID).
  3. Sign, date and post form to: LUCRF Super, PO Box 211, North Melbourne, VIC 3051.

Note: If you wish to rollover more than one account, you'll need to complete a separate form for each one.

Remember, before you transfer any super you should check any exit fees that may reduce the amount you rollover and see if any insurance or other benefits will be altered or lost.

What is certified identification?

Certified ID is when an authorised person sights your original document and a copy of it. They will then certify and confirm the copy of your ID by either stamping or writing 'certified true copy' followed by their signature, printed name and qualification. See example below.

Only certain people are authorised to certify ID, e.g. a Justice of the Peace or a pharmacist. For a full list of authorised people, please click here or refer to the Rollover to LUCRF Super brochure, see Related content.  

Authorised people who can certify your ID 

  • A Pharmacist
  • A registered Medical Practitioner
  • A Police Officer
  • A Registrar or Deputy Registrar of a court
  • A Justice of the Peace
  • A permanent employee of Australia Post with two or more years of continuous service
  • A finance company officer with two or more years of continuous service (with one or more financial companies)
  • An officer, or Authorised Representative of, a holder of an Australian Financial Services Licence (AFSL), having two or more years of continuous service
  • A member of the Institute of Chartered Accountants in Australia, CPA Australia or the National Institute of Accountants with two or more years of continuous service
  • A notary public officer
  • A Judge of a court
  • A Magistrate, or
  • A Chief Executive Officer of a Commonwealth court.
